Housing enquiry self-referral form

This form will be sent to the Homelessness Prevention Service upon submission.

It will be allocated to an officer within 24 hours Monday to Friday. An officer will contact you within 7 working days.

The form will take ten minutes to complete. If you do not submit the form no action will be taken.

Please do not complete more than one referral form as this will cause delays in the processing of your request.

The name and date of birth you provide will be validated against your identification documents. Please note that submitting false information is an offence.
